The Primary Care Network (PCN) connects primary care clinicians and directors across the Ottawa OHT-ÉSO to improve coordination of primary care, support clinicians' knowledge, and address challenges to clinician well-being. 

Are you looking for a family doctor or nurse practitioner?

Join Health Care Connect, Ontario's centralized waiting list for primary care. You can register online or by calling 811. To register, you will need a valid OHIP number and address in Ontario.

Transforming health through innovative solutions

Primary care is foundational in building a quality patient centred health care system, serving thousands of residents each day. Services are typically provided by a team of dedicated professionals, including physicians and nurse practitioners, committed to your health and well-being. 
It is critical to strengthen and integrate primary care services within the broader health care system if we are to improve health outcomes, enhance patient and clinician experience and promote the efficiency and equity of care. Think of primary care as your personal health care home. This is the place where you can stay healthy, receive support when you're feeling unwell, and connect with other essential health services within the system.

Advancing integrated primary care

The benefits and value of establishing a robust and inclusive network of primary care clinicians for each OHT, organized in a Primary Care Network (PCN), who can leverage their expertise to co-design system changes is foundational to advancing the priorities of the Ottawa OHT-ÉSO.
The PCN works collaboratively with the Ottawa OHT-ÉSO to strengthen the capacity of primary care to address the large number of unattached patients, improve access and attachment to comprehensive primary care, optimize care pathways, and address other local priorities, all with a focus advancing health equity.

Our Primary Care Network Leadership Group (PCNLG) helps to facilitate the connections and conversations between primary care clinicians in our Primary Care Network and partner organizations in the community to drive system changes. Join our primary care network

We want to work with you to make the changes that are important for people in our community. We need input, support, and direction from our family physician, nurse practitioner, and clinic director peers who are working in diverse primary care settings.

PCN members have access to accredited Lunch and Learn educational sessions, discounted medical and office supplies, support with coordinated funding proposals, and timely access to pertinent information in support of your practice.

Meet our Primary Care Network Leadership Group

Members are systems-minded, collaborative primary care clinicians who want to work with the primary care community and 65+ partner organizations connected to the Ottawa OHT-ÉSO to influence needed system changes.

The PCNLG was formed through an open call for members with a group of primary care peers supporting the selection process. PCNLG membership is comprised of family doctors and nurse practitioners representing diverse primary care models, career stages, racialised groups, skills and gender.
