The Client Partner Table (CPT) represents the voices of patients, families, and caregivers, ensuring lived experience informs decision-making, care pathway co-design, and the delivery of the Ottawa OHT-ÉSO's priorities.

Primary Care Network Leadership Group

The Primary Care Network Leadership Group engages and organizes local primary care to provide a collective voice in the Ottawa OHT-ÉSO and PCN.

Decision Support Working Group

This working group focuses on embedding performance measurement and evaluation within the OHT as well as increasing capacity for data-driven population health management across OHT partners.

ALC Working Group

The Alternate Level of Care Working Group (ALCWG) coordinates the Ottawa OHT-ÉSO and its health and community partners' efforts in developing and implementing equity-driven and patient centered initiatives to reduce new ALC designations and improve timely transitions to community care, aligning local priorities with regional ALC strategies.

Cancer Screening Working Wroup

The Ottawa OHT-ÉSO Cancer Screening Working Group brings together community partners to improve access to, coordination of, and participation in cancer screening activities, with a focus on equity-driven, patient-centred approaches that reduce barriers and improve early detection and improved outcomes.

Digital Health Leadership Table

The Digital Health Strategic Leadership Table (DH SLT) brings clinicians and system partners together to guide digital health initiatives that improve care, simplify workflows, and enhance patient outcomes across Ottawa. It ensures digital investments are practical, clinician-led, and aligned with provincial priorities to support a stronger, more connected health system.
