SCOPE is an evidence informed, virtual, interdisciplinary program that provides primary care clinicians with real-time navigation support, streamlined access to specific clinical pathways and specialist advice and a single coordinated point of contact.
Implementation of SCOPE in Ottawa is a collaborative initiative between the Ottawa OHT-ÉSO Digital Health Table, the Ottawa OHT-ÉSO Primary Care Network Leadership Group, and the Ottawa Hospital. The program currently supports 40 primary care clinicians and includes three established clinical pathways (digital imaging, ophthalmology, plaster). In 2026-2027, the program aims to onboard an additional 30 primary care clinicians and co-design two new clinical pathway.
