Older adults living with frailty

The Patient Navigation Monitoring Framework project is focused on developing a comprehensive framework that identifies essential evaluation criteria and key indicators to address both improvements and challenges in patient navigation. This initiative aligns with the priorities of the Ottawa OHT-ÉSO and supports broader regional objectives.

The framework centers on six critical measurement areas:

  • Experience with Care Transitions
  • Ease of Entry into the System
  • Collaborative Experience with Multiple Service Providers
  • Services Meet Needs in the Community
  • Information Pathways and Literacy
  • Experience with Navigation Support

This framework will serve as a valuable resource for applying project-level indicators consistent with the Quintuple Aim, aiming to foster a cohesive internal structure that enhances alignment and communication within the Ottawa OHT-ÉSO backbone team regarding navigation efforts.

Goals
  • Establish a patient navigation monitoring framework focused on improvement efforts and understanding navigation within the OHT network.
  • Ensure integration of the monitoring framework as a base principle into various projects to promote the development of navigation-related work.
  • ​Foster greater inter-OHT alignment and collaboration on a navigation measurement framework that evaluates key analytics on the integrated care experience for patients and providers.
