Lowertown health system redesign

In 2024, Ottawa OHT-ÉSO partners with primary care practices and community organizations in Lowertown and Sandy Hill to improve primary care attachment for residents facing barriers within populations experiencing current and historic social and health inequity. Lowertown and Sandy Hill was selected as the project location through an analysis of primary care attachment, primary care clinician location, and social determinant of health data.

This pilot program was successful in attaching over 200+ patients to primary care clinicians close to home. Individuals and families were matched with clinicians to ensure high-quality, long-term attachment where patients’ unique needs are met and clinicians are supported by interprofessional teams to sustain quality care over time.
