This self-assessment tool was originally developed by the Alliance for Healthier Communities. In collaboration with the Alliance and local partners, this toolkit was prepared by Ottawa OHT-ÉSO in 2024 to support the OHT and partners in identifying strengths, challenges, and and actionable steps to advance health equity in our community.
