Ottawa OHT-ÉSO IPCT Round 3 Announcement

On Tuesday, the Ministry of Health, in collaboration with Ontario Health, launched the 2027-2028 Call for Proposals to invest more than $250 million to attach 600,000 more people in Ontario to primary care. This is part of the Government’s $2.1 billion investment supporting Ontario’s Primary Care Action Plan which will expand or create over 300 new and expanded primary care teams across the province to attach two million more people to publicly funded primary care by 2029.

Again, OHTs will play a central role in working with local primary care clinicians and teams to develop and submit proposals that align with the four strategic priorities of this funding opportunity: 

  1. Primary care attachment 
  2. Readiness to implement 
  3. Supporting geographic coverage
  4. Meeting primary care objectives 

For the 2027-2028 Call for Proposals, there is no limit on the number of proposals OHTs and their PCNs can submit. However, proposals must take a data-based approach and be scaled and targeted to areas of unattachment, with an emphasis on collaborative and partnership-based proposals that support net-new attachment and improved access to primary care. Strong proposals should demonstrate meaningful collaboration among family physicians, nurse practitioners, IPCTs, OHTs, PCNs and other relevant health, community and social service partners. 

Like Round 2, in Round 3 proposals can be submitted to create or expand one of the following approved interprofessional primary care team models: 

  • Community health centres (CHCs)
  • Family Health Teams (FHTs)
  • Indigenous Primary Health Care Organizations (IPHCOs)
  • Nurse Practitioner-Led Clinics (NPLCs)

While Indigenous organizations are encouraged to collaborate with their local OHTs and PCNs, this is not a requirement to submit an Indigenous-led proposal. 

Submission deadline is Friday November 20th at 5:00 pm.
