The Ottawa OHT is committed to ensuring everyone in our area is attached to a family doctor, nurse practitioner, or primary care team by 2029. Achieving 100% patient attachment requires coordinated planning across our community.
To support this work, the Ottawa OHT has developed a Zone Planning approach, dividing central and south Ottawa into four planning zones to better coordinate primary care planning and collaboration. This approach was first used to coordinate proposals for the second round of Interprofessional Primary Care Team expansion funding, resulting in Ministry of Health funding for all six Ottawa OHT proposals. Building on this momentum, we will continue to bring together clinicians and primary care teams within each zone to strengthen relationships, identify shared priorities, and explore opportunities to work together to improve access to care.
