The Ottawa OHT-ÉSO’s is dedicated to increasing local capacity for data-driven population health management.

With the support of our Decision Support Working Group and data access partnerships with the Ottawa Neighbourhood Study and the Health Systems Performance Network (HSPN), we have developed a dashboard to:
1. ground health system planning in the health needs and social determinants of health factors of real communities in Ottawa
2. enable OHT partners to understand and plan responsively for the communities they serve

This dashboard was last updated in Fall 2025 to support planning for the second round of Interprofessional Primary Care Team funding applications. Please contact our Decision Support Lead, Shelley Horrocks ([email protected]) with any questions or concerns.
