Decision Support Working Group

The Decision Support Working Group is dedicated to enhancing data-driven decision-making within the Ottawa OHT-ÉSO. The group’s mandate is to:

  1. Support the embedding of performance measurement and evaluation within Ottawa OHT-ÉSO;  

  2. Increase capacity for data-driven population health management across Ottawa OHT-ÉSO partners by; 

    1. Promoting visibility of currently available health and social determinant of health data sources, 

    2. Creating opportunities to integrate, visualize, and report data at the ONS neighbourhood level to facilitate targeted population health management and impact measurement, and 

    3. Prioritize the flow of key health and social data for neighbourhood level population health management. 

Guiding principles focus on strengthening data collection and utilization, developing performance indicators, monitoring and analyzing health data, and facilitating effective communication between OHT-ÉSO and our partners. All activities are aligned with the Quintuple Aim Framework to ensure a holistic approach to healthcare improvement. The group also supports additional performance measurement-related tasks as identified by the OHT-ÉSO Collaborative Leadership Group.

  • you would like access to our validated list of client/patient intake questions to collect sociodemographic data for health and social service organizations.

The Ottawa OHT-ÉSO Decision Support Lead and Working Group have developed a number of decision support tools for partners to tailor programming to specific populations or local needs.
